For 2019-2020 School Year:

 

Please download the athlete packet from the attachments below and return to your athletic director. An emergency card is required with this packet, please ask for a blank emergency card from the attendance office.

If you would like to enroll online for the athletic insurance, please download application or go to the k12 student insurance website link below:

 

K-12 Student Insurance
 

(YOU MUST HAVE AT LEAST A 2.0 G.P.A TO PARTICIPATE IN ANY SPORT)*
Eligibility Dates are based on February Grades and April Grades (for playoffs).

To be eligible, you need the following documents:

1. Emergency Card
2. Insurance Application (see below)
3. "Parent Consent Insurance Code of Conduct" Form (see below)
4. "Physical Form" (see below)
5. "Steroid Abuse Form" (see below)
6. "Concussion Information Sheet" (see below)
*eligibility due dates are scheduled by the CIF-LA athletics department.
